Payer coverage · published in full

Every payer we cover, and every source behind it.

This is the list — all 84 payers and all 26,140 unique policy sources, nothing held back for a sales call. Each payer’s policy sources refresh on their own cadence and are flagged — not silently served — once they go stale. If a criterion in one of your appeals came from a source, you can open that source.

Why two numbers, and not one bigger one

Because counting the same document twice is how coverage numbers get inflated. 4 groups of plans on this list route to a policy corpus they share with their siblings — a licensee network publishing one set of medical policies and serving it under several brands. Counted per payer, that is 34,357 mappings. Counted per document, it is 26,140. We publish the smaller number.

The difference is 8,217 sources that would otherwise be counted more than once. It changes nothing operationally — a denial from a shared-corpus brand is answered from the corpus routed to that brand, and the citation names the document, not the brand — but it is the number you should hold us to.

How a source goes stale

Each source carries its own refresh cadence. When a payer republishes a policy, the source is re-indexed and the criterion’s refresh date moves. If a refresh fails, the source is marked degraded and the criteria drawn from it are flagged in the case — never served silently as current.

What isn’t here

If your payer mix includes a plan that isn’t on this list, say so in the pilot and we’ll tell you honestly whether onboarding it is a week or a quarter. New payer sources are added without a code deploy, but we won’t claim coverage we don’t have.

Now check ours against your denials.

Run it on your own denials. We return cited appeal packages and you judge the before-and-after in one meeting.
